Grace Laboratories is pleased to offer GRACE Antibody test in house for those who would like to have it done. Grace Laboratories will be perroming the testing under CLIA regulations. We can perform the testing at our facility for in and out of state patients if they are referred by authorized physician and also if your state does not have any additional certification requirements for out of state sample shipment and analysis.
Below you will find the Doctor's Requisition Form (fillable form) . Please click on the form and fill your information and print it out. Then have your primary care physian fill out his or her information and sign it. You may either fax the form or bring it with you to our facility after an appointment has been made.
For those whom can not be in person or out of state, you will find below the form that explains the proper way for Specimen Collection, Handeling and shipping procedures. There is a small fee associated with the test and Grace Laboratory can only accept cash or check at this time. All samples that are going to be shipped to Grace Laboratories will need to be paid by the shipper.
